Behavioral Health Case Manager

We’re hiring a Behavioral Health Case Manager to support our clinicians and monitor Talkiatry’s population of patients with mild to moderate mental illness.  You will play an essential role on the care team for our psychiatrists, psychiatric mental health nurse practitioners, and therapists.  They will rely on you to aid patients who require resources outside of therapy and medication management.  This includes identifying and connecting the patient with supportive services recommended by the patient’s therapist or psychiatrist.

When appropriate you will also play a central role in navigating a patient to higher acuity care settings.  Highlights of the position include the opportunity to work closely with psychiatrists as well as other medical professionals.  Your skills treating patients using evidence based psychosocial interventions such as motivational interviewing, behavioral activation, problem-solving, and distress tolerance will be essential. You have exceptional communication skills, are organized, and have outstanding follow-through skills.  This position offers a gratifying blend of both therapist and case manager duties.

You will:

  • Monitor the progress of an assigned population of patients 

  • As need is determined or requested by the patient’s therapist or psychiatrist, provide resource referrals and coordination. 

  • Participate in interdisciplinary care planning upon request 

  • Identify and address gaps in services and assist patients as necessary in accessing community programs and services, coordinating, and navigating as necessary 

  • Maintain patient records and communicate with patient’s care team (psychiatrist/therapist) and others as appropriate (PCP) 

  • Participate in coordinating crisis care, making referrals for inpatient hospitalization or emergency services as needed 

You have:

  • 3+ years case management experience with at least 1 year of experience in providing direct behavioral healthcare 

  • Bachelor’s degree required; Masters in social work, counseling or related field preferred 

  • Licensure in at least one of the following categories as required by state law: social work, counseling, LMHC, LCSW, LPC, or LAC

$75,000 - $80,000 a year

Compensation range for this position is$75,000 to $80,000, depending on experience; actual compensation will be dependent upon the specific role, location, as well as the individual’s qualifications, experience, skills and certifications.
